Covering Exposed Root Surfaces – Subepithelial Connective Tissue Graft

When gum recession results in root sensitivity to hot and cold foods and /or an unsightly appearance of the gum and tooth, connective tissue grafting can be performed to cover over the root. Connective tissue can be obtained from a patient's palate (from underneath the palatal skin) and placed onto the site with recession. Then an area of gum tissue is raised from the base of the recession in order to cover the gum tissue -- in order to provide a blood supply to the graft. The area of the donor tissue is covered with a glue bandage which virtually eliminates discomfort during healing. This procedure creates healthy, tough tissue and also create a better cosmetic result.

If the palatal site is not an option, an acellular dermal matrix (Alloderm) graft can be used as a donor source for the graft. The product is donated, medically-processed, human tissue. The advantage of this procedure is no need for a palatal donor site and the ability to replace a large quantity of tissue.

This technique can also be used for reconstruction of areas of tooth loss. When the tooth is removed and bone is not placed into the socket, an unsightly "depression" in the gum tissue forms. Too often, when a false tooth is replaced into the mouth, this "depression" requires the replacement tooth to be too long and unsightly. Before the replacement tooth is to be made, the graft should be done in order to reconstruct this deformed area. The result will be a more natural looking tooth and a happier smile (see ridge augmentation or extraction and ridge preservation section).

Coronally and pedicle positioned flaps, although technically not grafting procedures, are other forms of a pedicle grafts in that gingival tissue is freed up and moved either toward the top of the tooth or laterally. This requires adequate thickness and width of gingival tissue at the base of the recession defect or at the adjacent tooth.
