First Periodontal Visit Oak Brook IL

Please assist us at the time of your initial to our office by providing the following information:

  • Your referral slips and x-rays from your referring dentist (if not already sent)
  • A list of medications and dosages you are currently taking. This includes all over the counter medications as well, such as herbs and vitamins (please note that you can fill out your patient registration and medical history form on our website.) Please notify the office if you have a medical condition or concern prior to any procedures (e.g. artificial heart valves or joints), heart murmurs requiring pre-medication, blood thinners, diabetes, or hypertension.
  • If you have dental insurance, please bring in a CURRENT and CORRECT insurance card or form.

Please note: All patients under the age of 18 must be accompanied by a parent or guardian at the consultation appointment.

If you are a patient that has difficulty speaking or understanding the English language, please bring someone with you that can translate, this will make your appointment more beneficial for you and our office.

At our initial visit we will complete an in-depth medical and dental health history a thorough examination to measure for bone loss, loose teeth, bite, oral cancer screening, and other signs of periodontal disease.
